What is Tacrolimus test?

Tacrolimus (Tac) is an immunosuppressive drug that is used in preventing organ and tissue rejection in patients after transplantation. Tacrolimus test is a therapeutic drug monitoring (TDM) test that measures tacrolimus concentration—typically a trough sample taken just before the next dose—in EDTA whole blood to individualize dosing after transplantation and balance rejection prevention with toxicity risk.

  • Solid‑organ transplant recipients (kidney, liver, heart, lung) receiving tacrolimus, especially in the early post‑transplant period and whenever medications or clinical status change.
  • Patients with suspected graft dysfunction, rejection, or tacrolimus toxicity to correlate clinical findings with drug exposure.
  • Recipients starting or stopping interacting agents or at risk for food–drug interactions (e.g., grapefruit, pomelo), where levels may shift unpredictably.

Tacrolimus is an immunosuppressive agent used for prophylaxis of organ rejection post-transplant. Tacrolimus use is in combination with one or, most commonly, two other immunosuppressive medications. It has an application as an agent for the prevention or treatment of certain autoimmune diseases. In solid organ transplantation, it serves as the treatment of organ rejection in kidney, liver, and heart allogeneic transplants. There is also an off-label indication for the prevention of rejection in lung transplant patients.
