Triiodothyronine (T3) Test

The T3 (Triiodothyronine) test measures the level of T3, the biologically active thyroid hormone responsible for regulating metabolism, energy production, body temperature, heart rate, muscle function, brain function, and growth. T3 contains three iodine atoms and is produced partly by the thyroid gland, with about 80% formed by conversion of T4 (Thyroxine) in tissues such as the liver and kidneys. In the blood, most T3 is bound to proteins and inactive, while a small free T3 fraction is biologically active. The Total T3 test measures both bound and free T3. Because T3 is 3–5 times more potent than T4, it is the primary driver of thyroid hormone activity and is particularly useful in the diagnosis and monitoring of hyperthyroidism and T3 toxicosis.

Who should get this test done?

This test is recommended for individuals who:

●     Have abnormal TSH levels and require further thyroid evaluation.

●     Show symptoms of hyperthyroidism, such as weight loss, rapid heartbeat, sweating, tremors, nervousness, or heat intolerance.

●     Are suspected of having T3 toxicosis.

●     Are receiving treatment for thyroid disorders and require monitoring.

●     Experience unexplained weight changes, mood disturbances, cognitive difficulties, or cardiovascular symptoms.

T3 is 3–5 times more potent than T4 and is the main active thyroid hormone at the cellular level. About 80% of T3 is produced by conversion of T4 in peripheral tissues, a process that may be impaired in chronic illness, malnutrition, liver disease, or by certain medications, leading to Low T3 Syndrome (Euthyroid Sick Syndrome). T3 testing is particularly important for diagnosing T3 toxicosis, where T3 is elevated despite normal T4 levels. Since Total T3 can be affected by changes in carrier proteins, Free T3 (FT3) is often preferred as a more accurate measure of active thyroid hormone status.
