Hepatitis B virus (HBV) is a member of the Hepadnavirus family that causes an infectious inflammatory disease of the liver. Most individuals with chronic hepatitis B remain asymptomatic for many years or decades. Patients with HIV infection or post-organ transplant patients taking immunosuppressive drugs are at higher risk of developing chronic infection. However, patients with chronic hepatitis B are at risk of developing hepatocellular carcinoma (HCC). Viral DNA enters the nucleus and integrates into the host DNA immediately after infection. HBV replication initiates increased DNA synthesis and interferes with normal cellular detoxification and repair functions, causing chronic liver cell injury that leads to HCC.
Hepatitis B is also called as a silent epidemic because people simply do not have symptoms while being chronically infected or newly infected. Symptoms appear later when significant liver damage has occurred, ultimately leading to liver cirrhosis and liver cancer. Those who are silent carriers spread the disease unbeknownst to them. Since the liver has the capacity to function even when it is up to 80%, symptoms do not appear until there is significant liver damage.
