Skin Biopsy

A Skin Biopsy is a diagnostic procedure in which a small sample of skin tissue is removed and examined under a microscope to diagnose skin diseases, infections, inflammatory conditions, and skin cancers. Since many skin disorders appear similar on clinical examination, microscopic evaluation is often essential for an accurate diagnosis. The biopsy allows assessment of the epidermis, dermis, and subcutaneous tissue, along with any cellular or inflammatory changes present.

Who Should Get This Test Done?

This test may be recommended if individuals:

●     Have a suspicious skin lesion, changing mole, or pigmented lesion that requires evaluation for skin cancer, including melanoma.

●     Have a persistent rash, chronic inflammatory skin condition, or blistering skin disease that has not responded to treatment and requires a definitive diagnosis.

●     Have non-healing ulcers, recurrent lesions, nodules, or indurated skin changes that need assessment for infectious, inflammatory, or malignant causes.

●     Have skin findings suggestive of systemic diseases such as lupus, scleroderma, dermatomyositis, vasculitis, amyloidosis, or cutaneous lymphoma.

●     Have unexplained hair loss requiring a scalp biopsy to diagnose conditions such as alopecia areata, scarring alopecia, or other hair disorders.

Why Consider This Test?

Many skin conditions can look similar despite requiring very different treatments. A skin biopsy helps eliminate diagnostic uncertainty by revealing disease-specific microscopic changes within the skin.

It is the most reliable method for confirming or excluding melanoma and determining tumour depth, which guides treatment decisions. In autoimmune blistering diseases, histopathology and direct immunofluorescence provide definitive diagnostic information for timely treatment. Skin biopsy also helps diagnose systemic diseases such as vasculitis, sarcoidosis, and dermatomyositis through a relatively accessible and low-risk procedure.

Performed under local anaesthesia, skin biopsy is a minimally invasive and cost-effective investigation that can provide definitive answers and reduce the need for prolonged follow-up, imaging, or empirical treatment trials.

A Skin Biopsy is usually performed as a minor outpatient procedure under local anaesthesia. The technique used depends on the lesion:

●     Punch biopsy: Removes a cylindrical core of skin and is commonly used for inflammatory conditions and small lesions

●     Shave biopsy: Removes a superficial layer of skin and is suitable for raised lesions

●     Excisional biopsy: Removes the entire lesion with a margin of normal skin and is preferred for suspected melanoma and skin cancers

●     Incisional biopsy: Removes a representative portion of a larger lesion when complete removal is not feasible
