The Anti Smooth Muscle Antibody (ASMA) test is a blood test used to detect autoantibodies that target smooth muscle proteins, particularly actin. These antibodies are most commonly associated with autoimmune hepatitis (AIH), a condition in which the immune system mistakenly attacks the liver, causing inflammation and progressive liver damage. ASMA is one of the key markers of Type 1 Autoimmune Hepatitis, the most common form of the disease. Who Should Get This Test Done?

This test may be recommended if individuals:

●     Have persistently elevated liver enzymes (ALT, AST) or abnormal liver function tests without an identifiable cause.

●     Are being evaluated for autoimmune hepatitis or have a positive ANA test along with signs of liver disease.

●     Experience symptoms such as fatigue, jaundice, abdominal discomfort, itching, or unexplained weight loss suggestive of liver dysfunction.

●     Have another autoimmune condition and develop liver abnormalities, or have acute or chronic hepatitis with no clear infectious cause.

●     Have a family history of autoimmune liver disease, unexplained liver fibrosis or cirrhosis, or require monitoring of known autoimmune hepatitis and treatment response.

ASMA is commonly measured using indirect immunofluorescence (IFA), with results reported as positive or negative along with an antibody titre. Higher titres are generally more suggestive of autoimmune hepatitis than low-level positivity. Because ASMA can occasionally be detected in other liver diseases, infections, or even healthy individuals, results should always be interpreted alongside liver function tests, immunoglobulin levels, ANA findings, and, when needed, liver biopsy results. The diagnosis of autoimmune hepatitis relies on a combination of clinical, laboratory, and histological findings rather than any single test alone.
