The Complete PSA test includes total PSA, free PSA, and the ratio of free to total PSA. It is primarily used to screen for prostate cancer, assess prostate health, and distinguish between benign prostatic hyperplasia (BPH) and malignancy. It offers a more detailed assessment than the standard PSA test.

 It is generally recommended for men over 50 years old or younger men at high risk (family history, African descent). It may also be used for men experiencing urinary symptoms such as frequent urination, weak stream, or difficulty urinating.

The test requires a simple blood sample. No fasting is typically required, though patients should avoid ejaculation and certain medications for 48 hours before the test to ensure accuracy.

Elevated total PSA levels may indicate prostate inflammation, BPH, or prostate cancer. A low free-to-total PSA ratio suggests a higher likelihood of cancer, while a higher ratio is more often associated with benign conditions. The results must be interpreted alongside clinical findings and possibly followed by imaging or a prostate biopsy.

If results are abnormal, your doctor may recommend further tests such as MRI, digital rectal examination (DRE), or a prostate biopsy. Regular monitoring or a referral to a urologist may also be advised based on the risk stratification.
