This test simultaneously measures the cytomegalovirus (CMV) viral load and evaluates the presence of resistance mutations to the antiviral drug ganciclovir (GCV). It is crucial for monitoring CMV infections, especially in immunocompromised patients, and ensuring that antiviral therapy remains effective in controlling viral replication.
CMV is a common virus that can cause severe complications in transplant recipients, HIV-positive individuals, and newborns. Ganciclovir is a frontline antiviral agent used to treat CMV, but resistance can develop, leading to treatment failure. This combined test not only quantifies how much virus is present in the body but also assesses whether mutations in the UL97 or UL54 gene are present, which would reduce the efficacy of ganciclovir.
It is primarily used when there is suspected CMV reactivation in transplant patients or poor clinical response to ganciclovir treatment. It may also be part of routine CMV monitoring in high-risk patient populations. A rising viral load despite therapy often prompts this combined evaluation to guide alternative treatment strategies.
A high viral load indicates active infection or reactivation, requiring immediate clinical intervention. If resistance mutations are detected, it suggests that ganciclovir may no longer be effective, and alternative antivirals like foscarnet or cidofovir may be necessary. A low or undetectable viral load with no resistance mutations usually indicates effective viral suppression.
A blood sample is collected and analyzed using quantitative PCR for viral load measurement and genotypic sequencing to detect mutations. Results are typically available within a week, providing crucial data for ongoing treatment decisions in critical care settings.
