BK virus (Human polyomavirus 1) is a small, non-enveloped, double-stranded DNA virus of the Polyomaviridae family with lifelong latency in renal tubular and urothelial cells after primary infection in childhood. In kidney transplant recipients, reactivation can cause BK virus–associated nephropathy, often presenting as rising serum creatinine and may mimic rejection, making biopsy important for diagnosis.
