HLA A, B, DR LOCI TYPING is more preferred for solid organ transplantation whereas for marrow or cord blood transplant more detailed typing for C and DQ, DP is also done.

There are many HLA markers present in your body.

Research has found that a donor must match a minimum of 6 HLA markers. A close match is important because it:

• Improves the chances for a successful transplant.
• Helps your donor cells engraft (grow and make new blood cells in your body).
• Reduces the risk of complications like graft-versus-host disease (GVHD). GVHD happens when the immune cells from the donated cells (the graft) attack the recipient’s cells (the host).

HLA typing can also be done to determine relationships between children and parents when such relationships are in question.

HLA typing test has to be performed on the samples from the recipient and donor(s).

The genes encoding the HLA class I (A, B, and C) and the class II (DR, DQ, DP)  are the gene having most number of different combinations (alleles) some locations on the gene even have been found to have more than 300 varieties or alleles.

PCR or Polymerase Chain Reaction is done with both Donor and recipient blood samples to determine how much of their HLA gene class I and II are matching with each other.

Interpretation:

Depending on the results obtained, your doctor will be able to find whether your donor is right match for the transplant.  HLA is inherited from the parent. Brother and sister who shares the same parents has a 25% chance (1 in 4) of being a close HLA match. Extended family members are not likely to be close HLA matches. About 70% (7 out of 10) of patients who need a transplant won’t have a fully matched donor in their family.
